Identity Theft

DISH takes fraud and identity theft issues very seriously and is committed to protecting consumers against all forms of identity theft.

Inquiry Removal Request

To submit a request for removal of an inquiry from your credit file, please download and complete the form below.
(If you are filing an identity theft claim, you do not need to submit a separate inquiry removal request.)

As of June 2020, DISH Network does not post credit impacting inquiries to a consumer's credit file when inquiring for DISH satellite television services. These inquiries are considered soft and are not disputable but are available on your credit file as a reference.

Please FULLY complete the form. Without ALL of the required information, we will not be able to process your request.

Once you are ready to send your document, click the "Begin Claims Process" below and follow the prompts to upload your document.
